Narrow passage in bathroom

I need feed back or opinion on something I don't believe is specifically cited in the code, or am I wrong. Case in point and example; Upon entrance to a bathroom, the corner of the counter is approx 12" from the corner of the shower enclosure. BAsically, you have to squeeze through a 12" space to access bathroom. Obviously this is unacceptable, though is there an actual minimum measurement requirement?

If I recall correctly the minimum clearance in front of a plumbing fixture is 21", but I will need to verify that as it may be 21" in front of the toilet but only 18" in front of the lavatory.
